You can download the applicable SEQR forms as well as utilize the Environmental Assessment Form Mapper Application by accessing the NYS Department of Environmental Conversation’s website here.

Town Clerk & Tax Collector

Starting March 2025, older Vital Records information have been sent out to be digitized, so it may take 3-4 days to be retrieved. The search fee is $19. Some information has already been completed, so there may be no need for a search request, just call the Town at 315-536-8911 to see if the year of your request is in-house. If you are unsure if Milo has your information, a search can still be conducted, however the search fee would still be applicable if the records are not in-house. Please send your Application along with a copy of your ID, to process your search. If you would rather wait for records to return, please wait to submit your Application in September 2025.

Ways to submit your request:

Mail or drop off your application and copy of ID to 137 Main St, Penn Yan, NY 14527, or
Email your application and copy of ID to [email protected].

Applications for Certified Copy Requirements:

Marriage: Must be one of the spouses to apply; ID required
Birth: Must be person named on certificate, parents, or lawful representative; ID required
Death: Must be Spouse, parent, child or sibling of deceased or lawful representative; ID required
Cost: $10 per copy

Handicap Parking Permits Requirements:

For a Temporary Permit, an application with the Doctor's statement is required.
For a Permanent Permit, an application with the Doctor's statement is required.
Only 1 Handicap Permit per person
Cost: No Fee

Marriage Requirements:

Both parties must come in together.
Need 2 (two) forms of ID, any 2 of the following: Driver’s license/ID card, Social security card, Passport, Birth Certificate
Need to know your social security number.
Bring copies of all divorce papers and/or death certificate(s).
Must wait 24 hours but marry within 60 days.
Cost: $40 (we accept cash, check or card)
